Description

The first version of mUzima forms module will allow users to generate basic encounter form as well as registration form templates. To start off the mUzima form builder project, there's a need to create sample forms that can be used to demonstrate expected output from the form builder.

This ticket shall involve the creation of encounter forms as subtasks.

Update

The forms developed should be usable on the OpenMRS instance available at https://demo1.muzima.org. (username admin and password test).
