Create basic encounter form with at least 6 early signs for diabetes.

This ticket is for creating a sample encounter form with at least 6 early signs for diabetes. The form should have the 3 main sections of an encounter form.

The form developed should be usable (Successfully processed) on the OpenMRS instance available at https://demo1.muzima.org. (username admin and password test).
