Create Encounter Form with all vital signs

This ticket is for creating a sample encounter form with all the vital signs (At least 5 of them). The form should have the 3 main sections of an encounter form.

The form developed should be usable (Successfully processed) on the OpenMRS instance available at https://demo1.muzima.org. (username admin and password test).
