Create ability to invoke media capture from a form

There's need to create the ability to invoke the new media capture feature from within a form. The feature should work in such a way that if invoked from within a form, details of the captured media should be returned to the form so that they are made part of the form's payload.

This ticket should also involve creating a sample form that invokes the media capture.
