Create settings for concepts needed for media upload feature

There's need to create settings for concepts needed for media upload feature, that includes

• Media category: Captures uuid for a concept associated with the media category. The media category should be a convenient set that includes two set members: a coded concept for all possible answers (including a concept for Other non-coded ), and a concept for free text input of specific detail of the Other non-coded value.
• Uuid for the Other non-coded __concept in media category: ,
• Comments: Setting to specify the concept for capturing comments users provide related to media being uploaded
