Task: Create a tutorial documentation - Creating mUzima Form Templates

Steps:

• Export the current documentation from https://wiki.muzima.org/x/cIIU and make improvement by including the following
  • Create Encounter Type
  • Create Form Definition by using the new encounter type listed in previous step
  • Create New mUzima Form Template by using the new form definition.  You can use any existing form from mUzima Form Repository
  • Make sure to include the screenshots and description when creating new mUzima Form Template.    
• Demo Resource:  http://demo2.muzima.org/openmrs/login.htm (username: admin  password: test)
• Create a documentation with screenshots how the above are being done.  The documentation should include an introduction section what this document is about, a sub-section for each of the components, ending section should include "Thank you for your interests! For more information, please visit http://muzima.org/"
