Customization App Login Screen

      Description

      Currently, the app displays as mUzima Provider on Login Screen.

      We can consider to have customization in the following format so that implementation can set up something specific for themselves.

      1. Allow user to upload implementation logo (to replace mUzima logo)
      2. Allow user to replace mUzima for Health Providers (to implementation specific name)
      3. On the bottom of the screen, add "Powered by mUzima Provider Version x.x.x.)
