Registration Form lost entered value for other identifier field

After entering value for "Other Identifier" on mUzima Registration Form and saving it as incomplete or complete form data, the entered value for other identifier is gone after opening the incomplete form data.

As a user, I expect the entered value for other identifier on mUzima Registration Form would stay after it's being saved as incomplete form data and complete form data.

Acceptance Criteria:

• Enter value for other identifier field on mUzima Registration Form and save as incomplete form data, the entered value is still there after opening the incomplete for data.
• Enter value for other identifier field on mUzima Registration Form and save as complete form data, the entered value is still there after opening the complete for data.
