Unable to Save Dyplasia Form, Histopathology Form and Via Form

When you fill Histopathology Form and Via Form am unable to save it as "Draft" or "Complete" i get the error " Error occurred while saving Form". After filling the Dyplasia Form am able to save it as "Draft" but am unable to the save the form as "Complete" i get the same error message" Error occurred while saving Form"
