Apply validation against bounded to mUzima forms

Due to the changes in js file (see MUZIMA-251), it requires refactoring for all the current muzima forms which are affected by that change.

As a developer, I want to refactor and apply the change (on MUZIMA-251) to all the current mUzima forms, so that the same standard/validation applies to all forms.

Acceptance Criteria:

• Apply the validation against bounded to all mUzima forms
• Test it against the autocomplete form with valid outside of the options. It should prompt warning message "Please provide a location from the list of possible locations" or "Please provide a provider from the list of possible providers" accordingly.
