Remove name and description fields from muzima_form table

      Description

      mUzima_form table need to be updated to remove the name and description fields since those information are pulled from OMRS Form table and add retired field to mUzima_form table (when retired field in mUzima_table is ticked, it will retire the corresponding form_id from the form table). Need to update the REST API as well.
