Pull form definition from OpenMRS

      Description

      OpenMRS has form definition and mUzima Form Module also has form definition. They don't necessary match each other. It doesn't make sense to have the implementers to input the form definition more than once - on through OpenMRS form definition and one through mUzima Form definition.

      Ideal scenario:
      Pull the form definition (form name, version, etc.) from OpenMRS form definition and display them as mUzima form definition.
