Propose the UI design for the first mUzima forms builder

The first mUzima forms builder shall allow drag and drop of different types of HTML form fields to create a form in a WYSIWYG mode. The form builder should provide the following features:

1. Enable users to generate basic templates for mUzima registration forms as well as encounter forms
2. Enable users to edit basic form templates to customize labels on the forms, to include additional input fields and to add input validations into the forms
3. Enable users to preview created forms within the editor
4. Enable users to download/copy form template Html so that they are able to upload into their implementation OpenMRS instances.

The suggested UI could be an improved version of this form builder.

Update

• This ticket requires the creation of a user interface mockup that enables users to access the listed features, and as well the proposal of how the mockup can be actualised. The proposal should include information such as what development languages and what existing frameworks/libraries shall be reused where applicable. 
• The ticket allows more than one proposed mockup. Those who want to work on propose mockups should do it under subtasks available under this ticket.
• All Outreachy interns who want to be considered for the internship under mUzima in the December 2019 - March 2020 must attempt this ticket.
