Fix issues on Costing form identified during error resolution

During error resolution, the following issues were identified in Costing form, which require a fix on the form. This is based on the form key dated 17th July 2015 that is attached on this ticket

1. Question QD-5: Was the visit the result of any specific reason or disease e.g. malaria, typhoid, hypertension, pregnancy etc has the answers malaria, typhoid, hypertension, pregnancy instead of Yes, No, Don't Know, Refused
2. Question QD-6: Please tell me the reason(s)/disease(s) is set to use concept 9129 instead of 9178
3. Questions
4. Questions with the following concept IDs require changing values for Refused and Don't know from concepts 1624 and 1958 to -77 and -99 respectively, to be inline with concept dictionary where they have been defined as Numeric
