Refactor mPHR Architecture to a Modular Design

The team has determined that the monolithic architecture that the mPHR mobile application uses is not viable for a large set of users that are required to be pluggable, optional and reusable.

Therefore, the mUzima mobile architecture starting with mPHR will be refactored to use modules that can be downloaded by the user on demand.

This ticket is created to address the journey of refactoring the mobile architecture that is presently available to a modular one and as such develop modules for existing and upcoming features.
