Creation of duplicate person when converting person to patient via muzima app

The module creates an additional person when the conversion of a person is done from the application's relationship feature. This is due to an already existing record in the person table with the same UUID, thus the newly created person is assigned a new UUID and the mapping between new and old UUIDs done under the registration data table.
