Having duplicate patients on app

The app duplicates patients created on phone then later synced back to mobile, this is due to the server-side a new uuid being assigned to the patient and the app not able to download and consolidate the patient due to an issue with the resource that suppliers the records in the muzima_registration_data.

The application also crashes/closes if no identifier is given while deserializing.
