Update mUzima app to support defining relationships and picking GPS location within forms map

To support COVID-19 cases registration, there is need to:

• Enable users to pick GPS locations from a map as part of address information
• Enable users to define relationships within forms

To support this, there's need to enhance the app to enable:

• Searching persons from the app local storage and populating person details in the form
• Invoking the Relationships Person creation form from within a registration  form
• Invoking a GPS location picker from within a form
