FGH Tracing and Triangulation form Requests

There is a need to:

1. Add a javascript interface to allow for mUzima forms to query the cohorts in which a patient belongs to.
2. Delete patients who cease to be members of the downloaded cohort(s) as long as no incomplete/complete forms exist in the device
3. Tagging of patient on form save as complete (To enable providers to distinguish patients already traced/triangulated)
