Include settings in setup configuration

As currently designed, mUzima server-side settings are set globally. This may not work well for implementations that run several programs and that need to vary the settings across the different programs. For instance, one program may want to use the GPS feature while another program may not.

As a remedy to this problem, there is a need to make handling of the settings more flexible, by allowing implementations to specify settings within setup configurations. This should only apply to settings that may vary across programs of the same implementation.
