Always ask for retire reason when deleting items from mUzima core

To be synonymous with OpenMRS standards, there's a need to ask for reasons when users attempt to delete/retire items such as Setup Configs, Data sources, Cohort definitions. Currently, its only mUzima forms that ask for reasons when users attempt to delete.

This enhancement will help record audit information.
