Display form templates in the order in which they were created

To fill a form in mUzima, a user is usually presented with a list of available forms so that the user can choose the needed form. Currently, the forms are ordered alphabetically by their names. Some implementations have expressed the need to order forms differently using other criteria than alphabetical names.

A proposed solution is to display forms based on the order they appear in a setup configuration used to set up the device. In this case if any additional forms are downloaded to a device, the forms are displayed at the bottom of the list.
