Develop Diabetes Mini-Form

There is a need to develop a diabetes data entry mini form for mPHR patients.

This mini form should address the following data fields:

• The time the patient has entered new data 
• The last meal the patient has had.
• The time the patient had their last meal
• The blood sugar level at the time of entry

Logic

• **Trigger when the patient has entered critical data.
• Trigger when the patient has entered irregular data.

Trigger Actions

• **When the specified logic is triggered by either critical or irregular data show an appropriuate message and send the communication to the linked provider/clinician.
• The message shown to the user could include advice to visit the hospital.
