Develop mUzima Diabetese Enrollement Form

There is a need to develop a diabetes enrollment form for the diabetes use case implementation on mPHR and mUzima For Providers.

This diabetes enrollment form will also include mPHR install SMS submission facility for enrolling new patients into mPHR by facilitating them to install the mPHR mobile application.

Fields

• **Encounter Basic Information e,g date, location ...
• Patient Basic Information i.e Names, Age 
• Diabetes Enrollment Form Specific Fields such as; MELLITUS OR DIABETES INSIPIDUS  
• Define what is abnormal blood sugar'
• Define what blood sugar level should prompt going to the hospital.
