Liquibase optimization - adding preconditions if a table exists and character set to utf8

Currently: Some errors are encountered when loading the muzima module specifically on tables such as having existing tables and character sets not matching

Proposal: Have all table creation changesets include a precondition to exclude creation if exists, and include a changeset to update character set to utf8
