Forms allow invalid inputs

In Forms > Register Client(under the three dots) > Ampath generic form

• The country field should be a drop down.
• The names field allows entry of numbers and special characters.
• The location should be mentioned as Country-State-City/Village
• The phone number field allows entry of numbers and special characters.
