Changes and improvements on the CDM Health Centre encounter form

The following changes are to be effected on the current mUzima Health Centre form:

i) Display historical data (previous value) on question bearing "Status of patient" e.g

 - HTN Status, year of HTN Diagnosis,

 - DM Status, year of DM Diagnosis,

 - Current medication to display last encounter captured. (proposals were tabled on these feature and may need developers input)

ii) Move LMP field above Vitals section.

iii) Under problem list while specifying other, Diabetes and Hypertension options show up while checked by default.

iv) Add AM and PM dosing option for Glibenclamide since they could be different.

v) Add Lifestyle question with yes/no answers before the medication plan table with validation to have medication captured or lifestyle intervention.
