Fix app crash when no identifier type name is missing in generic registration form

      Description

      Whenever a patient record is registered using a generic registration form by providing an identifier value for an identifier type that has no identifier type name encoded in the form, the registration succeeds but the app crashes when a user visits the patient summary page.
