Display Potential Duplicates.

Right now the potential patient duplicates errors are mixed with other types of errors. The requirement of this task is to create a separate page that displays potential duplicates with ability to restrict matching to a particular attributes. (As checkboxes).

Clicking on an entry opens up another page displaying the two potential duplicates side by side.
