Capture GPS coordinates on mUzima Form Template

Currently, mUzima Form Template captures encounter location.  There is a need to capture GPS coordinates on the mUzima Form Template as well.

As a provider, I expect that the GPS coordinates will be captured automatically on my mUzima Form so that I don't need to tap on buttons in order to capture them.
