Create settings to allow customization of the landing page

As we add more feature to muzima, we need to allow the user to turn off some feature that the user won't use from the setting page. For example, user don't want to use the teleconsultation element, then they can remove the button from the landing page and the patient's overview page.

This would be good to work on once a plug-in module is ready.
