Unable to edit mUzima Form on mUzima Core Module 1.2.0

On mUzima Core Module 1.2.0, user is not unable to edit the mUzima Form once a previous form (with same name) was created.  

Steps to produce the error:

• Create a new mUzima Form (e.g. mUzima Demo Form version 1.0)
• Upload a newer version of the mUzima Demo Form
• It will prompt message that a form with the same name exists already.

Another way to produce the error:

• Continue with the above steps
• Go to Administration/Manage Forms, create a another name for the same form but with a newer version (e.g. mUzima Demo Form version 1.1)
• Go to mUzima Core Module/mUzima Forms, create a new mUzima Demo Form version 1.1
• It will prompt message that a form with the same name exists already.
