Create documentation to include the required elements for creating mUzima Form Template

As time goes, the required elements for creating mUzima Form Template have been growing.  It's time to have an updated documentation to clearly define all the required elements as well as creating a starter form that have all the required elements so that new members can use it for creating mUzima forms.
