MUzima core module should not allow uploading forms without form definition

Currently, it is possible to upload form templates without having selected corresponding form definition. This is problematic because whenever a form template is uploaded without the form definition, the module fails to load UI contents whenever users navigate to the mUzima forms tab. There's need to enhance the module to disallow uploading forms until form definition is specified.
