Add Relationship feature in mUzima App

There is need to relate/link clients in mUzima app. This should work the same way OpenMRS handles relationships with relationship types/options defined as person attributes or concepts.
An additional tab/section on the patient dashboard should then be added to show existing relationships for a patient with links to navigate to those related clients/patients

This ticket therefore calls for discussions on design and development of the feature as it is currently a blocker with HTS services in Kenya which requires linkages between clients and their partners/contacts.
