Display of coded observations and encounter types for data created on phone.

Observations created locally on device for for coded concepts are not rendered properly. An example of a coded observation answer not rendered properly looks as follows in the Obs viewer:

Also, Encounter types for encounters created locally use dummy encounter types named encounterTypeForObservationsCreatedOnPhone, which is annoying to users. There's need to

• Update coded observations to display names of coded values only
• Update encounters to use encounter types associated with forms
