Add ability to create setting in mUzima core, that defines whether primary medical record number is required or not

Add ability to create setting in mUzima core, that defines whether primary medical record number is required or not. This will be used by implementations to define whether or not to expect manual entry of primary identifier during client registration.

The default value for this setting is true, meaning that the identifier is required during registration
