Automatically create default data source in mUzimaCore module upon installation

Among other use cases, the data source feature was meant be used to identify implementations sending mUzima form data. This feature is however not fully developed.

Currently the server side module requires an entry in the data source table for it to accept data from the mobile app, while the app does not have logic to specify the data source. This creates confusion in the sense that users cannot upload data from mobile app until a data source is manually created.

By the look of things, this data source source feature will not be developed until there's substantial need for it.

To avoid confusion for new implementations, there's need to automatically create a data source during installation of mUzimaCore module.
