Add preference of encounter location under setting

Majority of time, the provider sees patients at specific location. It would make sense to have a setting to allow the user to have the preferred setting as defaulted encounter location so that users don't need to fill out the location everytime.

By adding the option under setting would allow users to decide whether they want to have it by default (default encounter location) or not.

As a provider on the device side, I want to have option to set the defaulted encounter location so that I don't need to update such info when filling out form templates.

Acceptance Criteria:
Login user can set the preferred encounter location for all form templates. If user doesn't want himself/herself to have default encounter location, he/she can uncheck the option under Setting.
