Long list of form definitions gets truncated at the 50th entry.

Muzima Core module upload forms function requires a user to select a form definition to attach the uploaded form to. Where there exists more than 50 form definitions in the implementation, only the first 50 are displayed with no way of choosing the others.

Requirement: The user should select any non-retired form definition in his implementation

Plan:

• If there are 10 or less form definitions, provide a combo box to pick a definition
• If there are more than 10 form definitions provide a search box to search for appropriate form
