Update existing registration form with the following fields:

• Ethnic Group: person_attribute_type_id=10
• Contact Person Name: person_attribute_type_id=11
• Contact Person Phone Number: person_attribute_type_id=12

Note: The person_attribute_type_id is based on LARK VM metadata.
