Error resolution UI does not list errors that have no value in provider and location columns, and cannot process data with no value in form_name column

Following recent updates, new columns - form_name, provider, location - were introduced in muzima_error_data, muzima_queue_data, and muzima_archive_data tables. Currently Error resolution UI does not list errors (from muzima_error_data) that have no value in provider and location columns, and cannot process data (from muzima_queue_data) with no value in form_name column.

There's need to update the processor module to allow display of errors that have no values in the listed columns, and handle situations where form_name is unknown.
