To Fix Issues on the Breast Cancer Screening Form v 0.01 Identified During Testing

The following are identified issues on the Breast Cancer Screening Form which requires a fix based on form testing using 19th Aug 2015 apk.

1. Date of Birth is not displayed
2. Under Gynecologic History the following are Hidden
(a) Age of first pregnancy
(b) Number of previous Deliveries
(c) Did you breastfeed your children
3. Physical Examination – Validate Respiratory Rate
4. Skin Exams Findings - When you select Mass Validate

(a) Mass measurement in cm, first direction
(b) Mass measurement in cm, second direction
5. When filling Provider System Id it should be displayed and not Hidden, you are only able to view Provider System ID after saving the form as complete or Draft.
