To Fix Issues on the Papsmear Form v 0.01 Identified During Testing

The following are identified issues on the Papsmear Form which requires a fix based on form testing using 19th Aug 2015 apk.
1. Date of Birth is not displayed
2. Enter future encounter date did not prompt warning immediately, it only prompts message when you save the form as complete “ Please enter a date prior or equal to today”
3. Enter future date for first day of last menstrual period does not prompt warning immediately, it only prompts message “Please enter a date prior or equal to today” when you save the form as complete.
4. When filling Provider System Id it should be displayed and not Hidden, you are only able to view Provider System ID after saving the form as complete or Draft.
