To Fix Issues on the Histopathology Form v0.01 Identified During Testing

The following are identified issues on the Histopathology Form which requires a fix based on form testing using 19th Aug 2015 apk.

1. Under Patient Demographics, it displays the form_uuid – it needs to be removed.
2. Gender “Female” should be disable as currently it still allows user to change to male on the dropdown box.
3. Date of Birth is not displayed.
4. Enter future encounter date it does not prompt warning immediately.
5. When filling Provider System Id it should be displayed and not Hidden, you are only able to view Provider System ID after saving the form as complete or Draft.
