      Description

      The following are identified issues on the Dyplasia Form which requires a fix based on form testing using 19th Aug 2015 apk.
      1. Date of Birth is not displayed
      2. Enter future encounter date it does not prompt warning immediately.
      3. Number of Previous Deliveries is hidden, it’s only seen after filling “Number of
      Previous Pregnancies” and tapping outside the field.
      4. The hint for “Number of previous deliveries” should be revised as currently it says
      “The number of children a woman has delivered.” Ideally, it should be one child
      per delivery but some delivery has multiple children. So, it is possible that a
      woman has five pregnancy with more than five deliveries assuming multiple births
      per delivery. Validate the field e.g “Please enter a value between 1-50”
      5. Enter future date for latest cd4 count date it does not prompt warning
      immediately, it only flags When you save the form as complete “please enter a
      date prior or equal to today” Validate the field e.g “Please enter a value between 500-1500
      6 ”Enter future date for most recent VIA result did not prompt warning immediately, it only flags When
      you save the form as complete “please enter a date prior or equal to today”
      7. Enter future date for most recent PAP result did not prompt warning immediately,
      it only flags When you save the form as complete “please enter a date prior or
      equal to today”
      8. Enter future date for most recent colposcopy result did not prompt warning
      immediately, it only flags When you save the form as complete “please enter a
      date prior or equal to today”
      9. Enter future date for most recent biopsy result did not prompt warning
      immediately, it only flags When you save the form as complete “please enter a
      date prior or equal to today”
      10. It has repeated section of “past treatment of dysplasia”. When clicking + sign, it
      opens up another section with the same answers selected in first section.
      11. Procedure Done Section – it allows user to pick both None and any other options.
      They should be mutually exclusive; user is able to pick none and other options.
      12. Is provider satisfied with quality of colposcopy done during today’s visit ? Extent
      of visualized lesion becomes blank. When you select “yes” and save the form
      extent of visualized visit becomes blank.
      13. Procedure Donewhen procedure done this visit is Biopsies, the previously
      entered o’clock becomes blank.
      14. Procedure Done  when procedure done this visit is Loop Electrosurgical
      Excision and Deep for Location of specimen the previously entered number of
      specimen collected becomes blank.
      15. Under management plan when you select Via six months follow up it should
      determine the RTC date.If you indicate RTC before or later than six months it
      should flag. Return visit plan should be based on the Management Plan.( Via
      6months follow up, Routine VIA in 1-year, Routine VIA in 3-years)
      16. When filling Provider System Id it should be displayed and not Hidden, you are
      only able to view Provider System ID after saving the form as complete or Draft.
