Include Location id against the location names during initial mUzima application wizard.

Currently during initial wizard in the step of downloading the Locations, the location names are listed without the corresponding location ids. The location names are so much confusing since some are similar hence making it difficult for the user to identify the correct location to download.

Acceptance criteria
The location id to be included against the location names to help the user distinguish and identify the correct location.
