Include provider system id against the provider names during initial mUzima application wizard.

Currently during initial wizard in the step of downloading provider, the list of names are displayed without corresponding provider system Id hence makes it difficult for the user to identify the correct provider in case of duplicate provider names.

Acceptance criteria.

The provider system id to be included against all the provider name to help the user distinguish and identify the correct provider to download whenever there are duplicate names.
