Error while Uploading Image on Teleconsultation Form

While trying to attach either a consultation image using the teleconsultation form, the form takes me back to top of the form erases all the fields I had filled and does not attach the media. It gives the message "There is one or more validation check failed on this form. Please review and resubmit the form This therefore makes me unable to save the form as complete.
