Unable to save form as it is having javascript error

Made changes to HtmlFormDataStore class in muzima-android as getProviderNamesFromDevice() was returning null string while reading provider names stored on the device. This error was causing the javascript to fail in the forms and the patient details were not getting pre-populated when the forms were opened.
